Patient safety, beware of medication errors!

Patient safety has become a priority worldwide and medication errors represent an important percentage of preventible adverse effects related to health systems. Although the incidence of errors in medication is low, the great number of patients attended to increases the number of patients affected. Diverse tools have been developed to detect errors, investigate and learn from them and to establish corrective measures. There are different elements that should be taken into account to improve safety.
